Editorial standards

Sourcing

Every factual claim links to a source. We prefer primary documents — SEC filings, CMS transmittals and fee schedules, Federal Register notices, DOL and EBSA reports, state Medicaid bulletins, payer provider manuals, court dockets, IRS Form 990s, and earnings calls.

Where we build on reporting by another publication, we say so and link to it. We do not rewrite someone else's story and present it as our own reporting.

We do not cite vendor blogs, content farms, AI-generated sites, or aggregators. If a claim cannot be supported by a primary document or an established trade publication, we do not make the claim.

Use of AI

We use AI in research and drafting. Every piece is reviewed by a human before it runs, and a human is accountable for what we publish. AI does not decide what we cover, and it does not publish anything on its own.

We state this because we would rather you hear it from us than discover it. It does not reduce our responsibility for accuracy by any amount.

Corrections

We correct errors promptly and visibly. Corrections carry a timestamp and describe what was wrong, not only what is now right. Substantive corrections are noted on the story itself and listed on our corrections page.

Data and methodology

Anything we publish that rests on a dataset carries a methodology note: what was included, the date range, definitions, and known gaps. Where our coverage is partial, we say so rather than implying completeness.

Right of reply

Where we report critically on a named company, payer or individual, we offer them an opportunity to respond before publication wherever practicable, and we publish substantive responses.
